• Certificate of Good Moral Character
• ESC Certificate/Voucher (if applicable)
• For incoming Grade 1: Early Childhood Development Checklist

Download from the Forms section, print, and prepare for submission during the Enrollment Schedule.

• Parent as Partners Contract Agreement
• Contact Information Form

Secure the following after payment

• Admission Slip
• School Calendar
• List of school supplies for distance learning
• Schedule of Training for Parents and caregivers
• Schedule of Dry-run of virtual instruction (2 weeks)
